In order to incentivise a needs-based response to crisis both in the short and long term, we should improve our ability to distinguish between severity that is reflective of two situations. The first situation is one of a more or less urgent need for immediate emergency measures to ensure survival and minimal access to basic services. The second is instead a lack of access to basic goods, services and infrastructure, or ‘deprivation’ more broadly, which is reflective of a more or less urgent need for the (likely not just humanitarian) response required to ensure a life in dignity. The longer a situation lasts, a response to both dimensions of need becomes equally important for a healthy life in dignity. However, in terms of their impact on people’s lives, as well as the response required, these needs likely remain very different. Laura Alessandretti and colleagues analyzed a dataset of 464,455 smartphone users from around the world, with 324,391 users categorized as urban and 52,290 categorized as rural. In most countries, urban users spent more time on their phones, using them for an average of 174.9 minutes a day, compared to 152.2 minutes for rural users. Urban users were more likely than rural users to spend time on apps categorized as maps and navigation (+150.0%), news (+38.7%), travel and local (+28.7%), music (+20.0%), business (+19.3%), and productivity (+13.7%) while rural users were more likely that urban users to spend time on apps categorized as weather (+29.9%), shopping (+18.3%), social (+8.8%), and game (+8.8%). Urban and rural users even tended to use different social media apps. During the 2015–2019 study period, urban users were more likely to use Instagram (+14.3) and Twitter (+24.2), whereas rural users favored Facebook (+17.8%) and Snapchat (+22.5). When database participants moved from the city to the country or vice versa, their usage tended to change to accord with their new surroundings, suggesting that it is environment rather than personality driving the differences. Our article describes what happens when a desert-adapted mouse in a simulated desert habitat is without access to water. It provides us a unique opportunity to explain the survival strategies employed by organisms in some of the harshest habitats on Earth. Desert mammals have been studied quite extensively for the last 100 years and given the current climate change reports, understanding environmental tolerance and the capacity for adaptation in one species can provide insight into the potential for similar species to respond to increasingly extreme climatic patterns that are likely to affect many habitats. We explored the physiological mechanisms that enable cactus mice to survive in desert habitats using long-term physiological data, observing that cactus mice have unique and important responses when water deprived. Without water they have significantly lower metabolisms, leading to reduced water loss compared to mice with access to water. Epistemology: Beyond psychological dynamics, some have identified epistemological deficiencies as a root cause for misinformation: issues with how users find and assess information especially online. Some have focused on online search or 'research' (Tripodi, 2018), behaviours, or looked more broadly at verification strategies (Schwarzenegger, 2019; Flintham et al., 2018). Expanding this focus on epistemology to the social level, others have ascribed the problems of misinformation to a broad national shift in the treatment of evidence and facts. Much of this work has drawn implicitly or explicitly on Foucauldian 'regimes of truth' (1980) or Jasanoff's 'civic epistemologies' (2004). The implication is that in recent years, the US has seen a broad shift both in the discourse around and mechanisms of how truth/falsehood is established in society. At the same time, this aligns with the long-recognised tactic by authoritarians to undermine the public's ability to sort truth from falsehoods (Arendt, 1951). Relatedly, many have ascribed the spread of misinformation, at least in part, to a broad reduction in trust in institutions. Public opinion surveys have tracked such a decline in trust across institutions for decades, and now, truth in most institutions is at historic lows (Pew, 2022). New research in the INFORMS journal Management Science finds that there are more than 7,000 rare diseases, but only 500 of them have a form of treatment. This study finds that governments can improve patient access to new drugs for rare diseases through government subsidies and innovative pricing schemes. The paper, “Improving Access to Rare Disease Treatments: Subsidy, Pricing, and Payment Schemes,” finds that government subsidies are crucial to improve the development and accessibility of new drugs for rare diseases. The researchers also find that having an independent consortium set the price can create social value. When exogenous pricing is supported by government subsidies, it can be effective in improving patient welfare and social welfare, while only slightly reducing the manufacturer’s profitability. Arguably the three attributes of General Practice which have contributed to its inherent value are continuity of care, ease of access and gate keeping. All three have suffered in recent years because of the workforce crisis, part-time working and the emergence of numerous alternative routes for patients into the broader health system. Once continuity of care disappears, as it has done in many Practices, the future of General Practice itself will be called into question. I joined the Stellar Development Foundation last year to lead the development of Stellar Aid Assist after Russia’s full-scale invasion of Ukraine. Stellar Aid Assist helps humanitarian organizations like UNHCR to send funds to people in need. Many humanitarian organizations operate cash and voucher assistance (CVA) programs as an alternative to providing physical goods like blankets and food. These programs allow recipients to have agency over their day-to-day lives, even in a time of crisis. They have been proven effective across the world. However, the legacy financial system presents hurdles to cash programs and interrupts the amazing work humanitarian organizations perform. Bank wires are expensive, slow, and only applicable to a portion of the recipient population. Money in transit is difficult to track. Delivering funds in physical cash can place recipients (and staff) at risk. The shortcomings of the legacy financial system disproportionately affect marginalized populations.
